Washington state disciplines health-care providers

State seal of Washington statePeriodically Washington State Department of Health issues an update on disciplinary actions taken against health care providers, including suspensions and revocations of ?licenses, certifications, or registrations of providers in the state.

The department has also suspends the credentials of people who have been prohibited from practicing in other states.

To find this information click on ?Provider Credential Search? on the left hand side of the Department of Health home page (www.doh.wa.gov).

The site includes information about a health care provider?s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998.

Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Here is the May 21 update issued by the Washington State Department of Health:

Note: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.

OLYMPIA???The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state.

The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department?s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Benton County

In April 2012 the Nursing Assistant Program indefinitely suspended the certification of nursing assistant?Konnie Lea Jones?(NC60232654) for failing to comply with an agreement to participate in a substance abuse monitoring program.

In April 2012 the Nursing Commission indefinitely suspended the license of practical nurse?John E. Vitkauskas?(LP60407036) for several instances where he failed to administer prescribed medications to nursing home residents in his care.

Chelan County

In April 2012 the Agency Affiliated Counselor Program indefinitely suspended the credential of counselor?Cynthia Lea Zellner?(CG60183248) for failing to comply with an agreement to participate in an substance abuse monitoring program.

Clark County

In April 2012 the Nursing Commission entered an agreed order with registered nurse?Natalie Alice Hendricks?(RN00113211) which requires her to complete continuing education in ethics, time management, legal issues, and nursing law. She was also ordered to pay a $2,000 fine. She worked as a nurse for two years on an expired credential. Her credential was renewed in November 20111.

Douglas County

In April 2012 the Nursing Assistant Program indefinitely suspended the credential of registered nursing assistant?Karen E. Flinchbaugh?(NA00158363) for failing to comply with an agreement to participate in a substance abuse monitoring program.

King County

In April 2012 the Department of Health suspended the credential of registered nurse?Mary Patterson?(RN00061617) for three years. She committed sexual misconduct with a patient.

In April 2012 the Pharmacy Board suspended the credential of pharmacy assistant?Barbara S. Gray?(VB60003049) for three years for attempting to divert controlled substances from her employer. Her credential is currently expired.

In March 2012 the Dental Commission entered an agreed order with dentist?Raymond K. Chan?(DE00005695) which requires him to complete continuing education in risk management and understanding why people abuse controlled substances. He was also ordered to reimburse $4,000 in costs to the commission and to pay a $15,000 fine. Chan?s prescribing practices didn?t meet care standards and he failed to maintain adequate treatment records.

In April 2012 the Massage Practitioner Program revoked the license of massage practitioner?Yan Chun An?(MA60214752) because of a criminal conviction for prostitution in October 2011.

In April 2012 the Massage Practitioner Program revoked the license of massage practitioner?Xiaoling Fei?(MA60178153) because of a criminal conviction for prostitution in December 2011.

In April 2012 The Pharmacy Board suspended the credential of pharmacist?Brian M. Johnson?(PH00009922) for two years. He dispensed controlled substances for his own use and tested positive for use of codeine and Oxycodone. He may apply to reinstate his license with evidence of participating in a substance abuse recovery program for two years and completion of coursework in pharmacy law.

In May 2012 the Department of Health ordered?Alex Harris?to cease and desist an unlicensed massage practice. He identified himself as a ?licensed massage therapist? and provided massages to clients for either cash or barter. He has never held a credential as a massage practitioner in Washington.

In May 2012 the Nursing Assistant Program indefinitely suspended the credential of registered nursing assistant?Derek C. Machel?(NA00105630) for failing to comply with terms of his probation.

Kitsap County

In April 2012 the Medical Commission entered an agreed order with physician?Lila Aflatooni?(MD00024955) who will surrender her license by December 31, 2012, while she is selling her medical practice. If her practice sells prior to that date, she will surrender her license at escrow closing and completion of another transition period agreed to as part of the sale. She also may not treat any pregnant woman or accept new patients who are pregnant. She must transfer her pregnant patients or those who become pregnant to the care of another provider. She was charged with failing to comply with an earlier order. She failed to treat a pregnant patient who had extremely high blood pressure. The patient saw another doctor the next day and was airlifted to the hospital.

Lewis County

In April 2012 the Nursing Assistant Program indefinitely suspended the credentials of nursing assistant?Bryan Scott Sabich?(NC60162801, NA60149516) for failing to comply with an agreement to participate in a drug monitoring program.

Pierce County

In April 2012 the Department of Health indefinitely suspended the credentials of paramedic and nursing assistant?Neale R. Porter?(ES01167098, NA00172269) for failing to comply with an agreement to participate in a drug monitoring program.

Snohomish County

In April 2012 the Nursing Commission entered an agreed order with registered nurse?Robert A. Krutenat?(RN00060280) to participate in a drug monitoring program. He was charged with stealing controlled substances from his employer for his personal use.

In May 2012 the Chemical Dependency Professional Program indefinitely suspended the certification of chemical dependency professional trainee?Allison N. Alexander?(CO60253336) for failing to comply with an agreement requiring participation in a substance abuse monitoring program.

Thurston County

In April 2012 the Nursing Assistant Program suspended the certification of nursing assistant?Lynn Knutz?(NC10101466) for two years for failing to comply with an agreement to participate in a substance abuse monitoring program. Her credential expired in April 2008.

Yakima County

In April 2012?Jason R. Whiley?(VB60076882) surrendered his pharmacy assistant credential to the Pharmacy Board. He diverted controlled substances from his place of employment on at least 20 occasions. His credential expired in December 2011 but he was eligible for late renewal.

In May 2012, the Department of Health ordered?Karyen Thompson?to cease and desist practice as a physician or medical doctor. She offered to cure and treat human diseases such as pancreatitis and diabetes. Between 2008 and 2011, she impersonated a medical physician in Washington and was convicted of first degree criminal impersonation in 2011.

In April 2012?Betsy Ann Raap?(LP00040265, RN00110272) permanently surrendered her nursing credentials to the Nursing Commission. She diverted drugs and/or failed to account for withdrawn medications.

Out of State

Florida:?In April 2012 the Nursing Commission suspended the license of registered nurse?Mary Lee Delate?(RN60020122) for three years and fined her $1,000. Her license expired in July 2011 but was eligible for renewal. She was convicted of conspiracy to possess with intent to distribute a controlled substance.

Indiana:?In April 2012 indefinitely suspended the credential of registered nurse?Carrie Ann Withey?(RN60150817). Her Indiana nursing license was suspended by the Board of Nursing for diverting controlled substances from her employer for personal use. Her Washington license was immediately suspended in March 2012.

Minnesota:?In April 2012 the Nursing Commission immediately suspended the credential of registered nurse?Theresa M. Welna?(RN00131021). In February 2012 the Minnesota Board of Nursing suspended her nursing license based on a finding that she diverted controlled substances from her employer for her personal use.

Minnesota:?In April 2012 the Nursing Assistant Program indefinitely suspended the credentials of nursing assistant?Pia Loraine Rehnberg?(NA60051169, NC60090468). Her credentials are expired by eligible for renewal. She was charged with alcohol abuse while working.
